This application form should be completed by the applicant if aged 18 or over, or by a parent / guardian if the applicant is under 18 or is a vulnerable adult.

A note about crew selection: To ensure the safety of all embarked crew and staff and on some of our voyages we have to ensure that our participants are suitable for the voyage they want to undertake; we may have to reject applicants if we feel they will cause a risk to the vessel, its crew or themselves. We will always discuss this with you prior to rejecting your application.

Part 1 - Participants personal details
Part 2 - Health, Diet, & Swimming Ability

It is important you declare all medical issues; no-matter how minor so we can ensure the participant will be safe aboard.
If fit and healthy please declare this in each field.

Part 3 - Emergency Contact Ashore

Please supply the contact information for a next of kin or emergency contact; this ideally, will be the parent or guardian of the participant. They should be contactable at all times whilst the participant is aboard. Please note, for safety reasons this cannot be someone who is sailing with you.

Part 4 - Declarations

These declarations must be made by the applicant if aged 18 or over, or by the parent or guardian if the applicant is under the age of 18 or is a vulnerable adult.

Part 5 - Media Preferences

Sharing the special moments that happen aboard is massively helpful in promoting the aims and objectives of the Project, especially in being able to record moments to show prospective or current sponsors, funders, or supportors. We need your persmission to take pictures and/or feature you or your child in video we take aboard. This can be withdrawn at any time. People shown in pictures or videos will not be named. We use Facebook, Twitter & Instagram social networking sites.
